There are about 600,000 themes, they all look at least a little different and they all do something a little different. This question is far too broad.

Some change the icons from the ones on the springboard to the wifi icon or the slide to unlock text to something else and usually they come with their own set of sounds and animated lock screens and wallpapers, there are tones of good ones out there
