There is one thing I’m not clear on regarding iOS backups across these three methods:
iTunes
Finder
iCloud
.


Does an iPhone backup consist of a block-by-block copy of iOS? In other words, is an 'image' of iOS generated?
If that were the case, I imagine the entire operating system would be copied along with the current configuration.
If I remember correctly, option 3 (iCloud) does not back up the OS.
What way can I use to install fresh and cleaned installation of iOS ?

None of those backups include the operating system. If you want to install the latest iOS, you can restore the iPhone to factory settings and it will automatically install the latest version.

thanks, so best solution is to make factory reset and I can restore all apps and its data by restoring from iCloud backup ?
