Unlock iPhone4

I have been using unlocked iPhone4 with OS 4.0.1.

SO If I upgrade OS to 5 do I need to unlock it again or not required?

if you decide you want to upgrade then you will need to use redsn0w and create a custom firmware that will allow you to then preserve the un-lockable baseband and upgrade in itunes to a jailbroken state where you can then unlock with ultrasn0w, unless you used a hardware solution such as gevey?

You'll need to preserve your Baseband by updating w/ a custom IPSW. Are you on BB 1.59.00 & using the ultrasnOw unlock?

As Unleashed said above... you can use redsn0w to create the custom IPSW. Or you can use sn0wbreeze to do the same.

No...I did it from a mobile store...

Yes BB is 1.59 and used UltrasnOw first time for unlocked..
