My wife's iPhone 4 was mistakenly connected to my daughter's iTunes account. After endless attempts at finding the correct username/password combo, I need to just start fresh. How can I associate her phone with an entirely new iTunes account? At present, she cannot update the phone or the apps. Help!

You can't associate an iTunes account to an entirely new iTunes account specially if you have purchased items in the old iTunes account. You need to know the user name and the password of the old iTunes account to update the apps purchased using the old iTunes account even after successfully setting up the new iTunes account on your computer and on your iPhone.
I believe that Apple gives several options to recover iTunes accounts passwords, all you need to provide to Apple is your email address you use to login your old iTunes account and then Apple will send you a link allowing you to reset your iTunes account password.
