It looks like you've enabled call forwarding.

It can be disabled in Settings - Phone - Calls - Call Forwarding.

That's a call forwarding icon. If u didn't activate it, it may be a "bug" with your phone. Try doing a hard reset by holding the home button and the power button down at the same time until the apple logo appears. Then release and wait for your phone to reboot
