Urgent iphone help

hey, so yesterday night my iphone 4 got plugged into charge but it came up with the fully charged symbol even though it was on 5%, i thought nothing of it so i let it drain then put it on to charge and it came up with the big battery and lightning bolt. i came back an hour later and this was still there so the battery had not charged past that point. i tried to plug it into my laptop to restore it but nothing happened and tried hard resetting it but it only came up with slide to power off. i restored it on the actual device and chose restore as a new phone, i did this but it said i need to connect it with itunes which i cant do and as soon as i take it off the wall charger it goes flat straight away, i downloaded the newest version of itunes and my iphone had the newest version of software but its still not working. please help! thank you

Have you try different charger? If yes, look like your battery not doing well.
