Unable to Delete a reminder icon from my phone

I have an orange color reminder icon in addition to the native reminder icon on my phone and i am unable to delete the same for reasons unknown to me

Can someone please suggest a solution. I remember having installed the same from cydia and might have removed an application associated with it.

I am using a jailbroken iphone 4s with ios 6.1
