There's probably an app for this but I did it with a simple wallpaper. This wallpaper adds aesthetic value to the lock screen as well as useful information. How to set it up:
1. Download the wallpaper
2. Enter any information you desire using an image editing program - Photoshop or GIMP work great
3. Save the image, transfer it to your phone
4. Set the new image as your lock screen wallpaper: Settings > Wallpaper > (choose image) > tap the Set button > Set Lock Screen
I use this wallpaper to show my name and an email address in case anyone finds my phone - hoping that the person who finds it is honest.
