My Apple ID shows up on my wife's phone

How do I get my ID off her phone? Ive tried resetting her phone and Wiping it clean. But to no avail, problem still persists.
When she goes to add an APP it sometimes asks for my ID and sometimes asks for hers.

You mean name of appstore id

If you mean that

Go to app store > top 25 > scroll down

You will see apple id

You can forget and other setting

gonto featured tab. scroll to the bottom. see your apple id? click that. pop up menu comes out. click sign out.
