app to fill an online form

has anyone seen or built an app that will fill an online form? I have a website that is used to submit calls to a workflow and am trying to build an application that stores some data on the phone for the user, and allows them to enter and queue the event. However, would like it to simply dump all the data to the online system, and use that as the system for processing.
anyone done this? probably makes more sense as a well developed mobile website but dont have access to the right servers etc. to make that happen.
