Personal Hotspot can be disabled by a providers OTA update for towers.

Try contacting your cellular provider, they may have to re-activate it for you.

I was not aware a PRL update would also possibly change other settings. That's interesting.

I wonder what else they can change.

With iOS 6 they went beyond PRL updates. They now can update everything concerning the radio in a modular format.

Contact your cell provider, it is controlled by them.
